DESCRIPTION:
Coordinate and provide community opportunities for adults with I/DD.
Plan and implement regular training activities including social, health, safety, transportation, recreation, vocational, and volunteer programs.
Assure scheduling of direct support instructors that meets participants’ goals and needs.
Supervise staff providing services and ensure timely completion of all documentation in EHR system.
Responsible for program implementation, documentation, and completion of billing/eligibility criteria in accordance with funding and accreditation standards.
Ensure implementation of individualized support plans.
Maintain effective working relationships with service participants, families/caregivers, staff, funding sources, community agencies, and others as appropriate.
QUALIFICATIONS:
Bachelor’s degree in human services or related program and two years of experience providing services to adults with intellectual developmental disabilities, or an equivalent combination of education and experience required.
QP status required.
Must have experience with I/DD Medicaid funded services.
Ability to work in a team environment, supervise and train staff, and coordinate service delivery and plan meetings.
Ability to exchange information and to communicate progress and needs of participants in a variety of settings.
Must be able to actively assist participants in varied activities and locations related to service delivery.
Computer and EHR proficiency required.
Background check, MVR check with acceptable driving record, and reliable vehicle required.
DETAILS:
Full-Time, Exempt.
Monday-Friday, daytime schedule.
